VIRTUAL APPOINTMENTS

Virtual Appointments allow patients to connect with your healthcare provider remotely through a secure video visit. This convenient option makes it easier to receive care, discuss health concerns, review test results, and follow up on treatment plans. Virtual visits provide a flexible way to stay connected with your provider while maintaining continuity of care.

REMOTE PATIENT MONITORING
Remote Patient Monitoring services help patients track blood pressure monitoring, glucose monitoring, along with medication reminders to support adherence to prescribed treatment plans. Patients also can receive monthly check-in calls from a care coordinator to review their health progress, address concerns, and provide ongoing support. 

ONLINE PATIENT PORTAL – ELATION PASSPORT
The Elation Passport Patient Portal provides a secure and convenient way to stay connected with your care team and manage your healthcare online. Through the portal, patients can send messages to their care team, schedule appointments, request prescription refills, and review your medical history.
