Step 1: Access the Scheduling Portal
Log into the Elation EHR patient portal or mobile app. You may also access online scheduling directly by visiting: https://app.elationemr.com/book/tworiversfamilypractice

Step 2: Select Appointment Preferences
You will see three dropdown menus and a list of available appointment types and times. You may scroll through the list or use the filters instruction below to narrow your search:

 

1. Provider: Select your preferred provider from the dropdown menu.

For services such as B-12 injections, urinalysis, vaginal swabs, or testosterone injections, please select “Nurse Visits.”

2. Appointment Type(s): Choose the appropriate appointment type(s) by selecting from the dropdown list. 

• You may select up to two appointment types per visit. 

• Annual exams must be scheduled at least 12 months apart. 

• Unselect any options that do not apply. 

 

3. Time Zone: Confirm that the correct time zone is selected. 
Available appointment times will update based on your selections.

Step 3: Choose Date and Time
Select your preferred appointment date and time from the list provided.

• The next available appointments will appear first. 

• To view additional dates, click the “More” button.

Step 4: Confirm Appointment
Review your appointment details.

• If you are not already logged into the portal, you will be prompted to log in via the Elation EHR patient portal login button. 

• After logging in, confirm your appointment information to complete the booking. 

 

Step 5: Complete Pre-Appointment Forms
Please complete any required forms sent to you prior to your appointment.
