MEDICAL CANNABIS CARD
Two Rivers Family Practice offers evaluations for Medical Marijuana Cards for qualifying conditions. The doctor at will assess your medical history, symptoms, and conditions, as well as discuss the possible benefits and potential health risks involved.

If approved, your qualified physician issuing the medical marijuana recommendation submits it to the Medical Marijuana Use Registry, on the same day. You’ll receive an email to finalize your online application. Renewal evaluations are also offered and can be provided via telehealth for established patients.
