NOTICES OF PRIVACY PRACTICES

We are committed to providing quality health care and respecting the privacy and confidentiality of your medical information. Our policies and procedures conform to all state and federal laws.

NEW PATIENTS

We keep very accurate medical records for you. To aid us in this each of the following forms must be completely filled out and signed by you as a new patient prior to your first visit.

The following new patient forms are supplied for your convenience. They may be downloaded and filled out to bring in for your first visit. If you are unable to fill out these forms prior to your initial visit, we ask that you come to your appointment 20-30 minutes early to be able to fill the forms out completely prior to you receiving care or treatment. Forms CANNOT be faxed or e-mailed. All forms must be filled out legibly and in the format provided or you may be asked to re-fill them out at the office.

NEW PATIENTS PLEASE NOTE

If you do not see your insurance carrier listed on our insurances accepted page we are able to verify coverage for patients Please phone our office and ask for Isabel. Please be prepared to provide the insurance company Name, Insured Name and ID# billing address for the card holder. We are then able to check this on our billing website.
