COVID-19 Safety Protocols
We continue to follow guidelines set forth by the Center for Disease and Prevention (CDC) and the Florida Department of Health Services (DHS). We are taking additional precautions to keep our patients, our providers and our staff healthy and safe.

PATIENT SCREENING PROCEDURES

Face masks are now OPTIONAL.

While masks are not required, kindly wear a mask if you have potentially been exposed to COVID-19 or if you feel unwell (cough, fever, sore throat, fatigue, body aches, or shortness of breath) or have any exposure concerns.

Safe social distancing is practiced in our waiting rooms.

Patients may wait outside the office or in their car, if that is preferred over waiting inside the office.

Hand sanitizers and wipes are readily available throughout the office.

All reading material has been removed from the waiting room and exam rooms.

Every exam room is cleaned and disinfected after each patient visit.

We keep very accurate medical records for you. To aid us in this each of the following forms must be completely filled out and signed by you as a new patient prior to your first visit.
The following new patient forms are supplied for your convenience. They may be downloaded and filled out to bring in for your first visit. If you are unable to fill out these forms prior to your initial visit, we ask that you come to your appointment 20-30 minutes early to be able to fill the forms out completely prior to you receiving care or treatment. Forms CANNOT be faxed or e-mailed. All forms must be filled out legibly and in the format provided or you may be asked to re-fill them out at the office.
