PHYSICAL REHABILITATION
At Two Rivers Family Practice, Dr. Homer serves as the Functional Assessment and Rehabilitation Coordinator, embedding structured mobility, balance, and gait assessments directly into primary care workflows. His role strengthens Annual Wellness Visits, chronic care management, and post-hospital transitions by identifying fall risk, mobility decline, and functional barriers before they become injuries or hospitalizations.
Dr. Homer specializes in translating complex movement science into practical, patient-centered care. His work focuses on gait, balance, fall-risk, lower-extremity function, and functional independence—particularly in aging and medically complex populations. He has directed motion analysis and rehabilitation programs, developed evidence-based screening protocols, and collaborated with physicians to enhance clinical decision-making through objective functional data.
